What Information Belongs on a Waxing Consultation Form?

A waxing consultation form can help you explain the area you want treated, relevant medicines and skin products, current skin concerns and previous reactions. Answer specifically, ask why information is needed if it is unclear, and tell the practitioner when something has changed. Completing a form is not a guarantee that treatment is suitable.

Why the exact medicine or product matters

“Acne treatment” or “anti-ageing cream” may not identify the product clearly enough for a preparation discussion. Record the name, active ingredient if known, where you apply it and when you last used it. Bring the packaging or ask a pharmacist for help if you cannot identify the product.

AAD notes that some products, including retinol, prescription retinoids and benzoyl peroxide, may make skin more sensitive or cause peeling 1. Its waxing guidance advises against waxing if isotretinoin has been taken within the previous six months, and gives a two-to-five-day precaution for retinol and prescription retinoids before facial waxing 2 3.

Do not use a beauty form as an instruction to stop a prescription. Ask its prescriber about the planned waxing and share relevant advice with the practitioner. A generic question cannot establish whether a particular area is safe for you.

Describe the area and what you have observed

State which area you want treated and which boundary you want left alone. If the treatment name is unclear, ask what it includes before choosing it.

Mention current irritation, tenderness, peeling, sunburn or broken skin. AAD advises against waxing sunburned or very sensitive skin 2. If you have had a previous reaction, describe the product or treatment, what you noticed and how long it lasted rather than giving yourself a diagnosis.

Hair-removal history can also be useful. Note the method and date, particularly if different areas were treated differently. AAD gives a general waxing hair-length range of about one-quarter to three-quarters of an inch, approximately 6–19 mm 2. Treat that as preparation guidance to discuss, not individual clearance or a product-specific Luté rule.

Ask what the studio actually needs

If a question feels unclear or more personal than you expected, ask how it relates to the proposed treatment. You can ask to discuss a sensitive detail privately and ask what information is necessary before deciding what to share.

Do not assume that every studio uses the same form, a particular changing room or the same record system. Luté Beauty Studio's public contact and price information does not establish an internal intake or medical-clearance protocol. The studio lists its location as Lilayi, Lusaka and its phone and WhatsApp number as +260 772 655 231 8. Use the actual contact route to ask about arrangements.

Questions about records

These are useful questions to ask; they are not statements of Zambian law or a description of Luté's adopted policy:

Your consultation-question notes

Use this personal preparation aid rather than treating it as an official studio screening form.

QuestionWhy it may be usefulYour note or follow-up
What area and boundary do I want?It makes your request clearer.______
Which medicines or skin products do I use there?Relevant precautions may need discussion with the prescriber and practitioner.______
Is there a current skin concern?Sunburn or very sensitive skin should not be waxed under AAD guidance 2.______
Have I reacted to a product or treatment before?Specific observations are clearer than a broad label.______
When and how did I last remove the hair?It records your actual preparation history.______
What do I want to know about privacy or records? You can ask before sharing sensitive details.______

Review the current treatment menu, choose the area you want to discuss and keep unanswered questions beside your booking details. A form should support a clearer conversation, not replace one.
